A faulty Toyota is being blamed for yet another deadly crash, this time involving a vehicle that had already been recalled. Investigators believe an accelerator problem on a Toyota Camry was the likely cause of a crash that killed two people in Utah and injured two others, AP reports. The vehicle had been subject to at least three recalls, including a mandatory one to fix a sticking accelerator, which investigators have confirmed was completed.
Investigators are trying to determine whether two voluntary recalls and repairs for a short accelerator pad and a sticky floor mat were also completed. Police say the driver, who was killed in the crash, tried and failed to stop his 2008 Camry as he left an interstate highway and crashed into a rock wall. His son's fiancée was also killed. His son and wife were injured. The brakes were in working order and skid marks at the scene indicated that the driver had tried to use them.
